          A 29-year-old man developed his first seizure 2.5 years prior to

          his death. A CT 6 months later howed a right cerebral mass. He

          continued to have seizures and eventually developed papilledema.

          A biopsy 14 months prior to death showed low grade astrocytoma.

          He continued to do poorly in         spite of radiation and chemotherapy   in Houston, Texas and died 1 month after another biopsy.

          Dx: gliomatosis cerebri

          A 6 year-old boy was born with meningomyelocele which was

          resected soon after birth. He developed hydrocephalus which

          was treated with ventriculoperitoneal shunt. He showed scolisis,

          neurogenic bladder and bowel, bilateral IX, X and XI cranial

          nerve palsy and nocturnal cardiac         dysrhythmia. A pacemaker was           placed. He died of febrile illness at age 6.

          Dx: Arnold-Chiari ( Chiari II ) malformation.

          A 37-year-old HIV positive man developed incontinece and mental

          deterioration followed by right         hemiparesis 6 weeks prior to

          death. CT showed a large enhancing lesion in the left basal

          ganglia. He also developed herpetic lesion in the groin and

          diarrhea. Died of respiratory arrest shortly           after admission.

             Dx: Lymphoma, left striatum, AIDS.

          A 52-year-old HIV positive man developed burring vision followed

          by left hemiparesis and mild cerebellar symptoms 6 months before

          death. MRI showed lesions in the right frontoparietal and left

        occipital white matter. He died of staphylococcus pneumonia.

             Dx: progressive multifocal leucoencephalopathy ( PML ), AIDS.

          A 18-year-old female had been bedridden with cerebral palsy. 

          She was retarded. She learnt a few words but lose them.

          Dx: cerebral calcinosis, Cockayne's disease.

          A 82-year-old man had been demented after a head injury 12 years

          ago. He also had a history of abdominal aortic aneurysm repaired

          and myocardial infarct in the past.  He died of septic shock.

          Dx: old cerebral contusion, etat crible, post-traumatic dementia.

          A 35-year-old male had left frontal oligodendroglioma resected

          in 1974. The tumor was called high grade astrocytoma by other

          consultants in Palo Alto and San Francisco. He was treated with

          6600 rad irradition. He was re-operated in 11-89 for recurrence.

          MRI in 10-91 showed local recurrence and dissemination of the           initialresection.

          Dx: recurrent and disseminated mixed glioma, now glioblastoma.

          A 50-year-old chronic alcoholic male was found unresponsive.

          He was operated on for right subdural and intracerebal

          hemorrhages after MRI. He remained in coma for one year before

          he died.

          Dx: old cerebral contusion, diffuse leucoencephalopathy,

                  brainstem compression with  hydrocephalus.

          A 64-year-old man was treated with irradiation for an inoperable

          tumor in the sellat region in 1985. He was well for 4 years, then

          developed progressive mental deterioration. He was found

          unresponsive with panhypopituitarism and increased intracranial

          pressure. He was treated with a shunt but died 2 days later.

          Dx: Radiated craniopharyngioma, necrotic and gliotic hypothalamus.

          A 35-year-old woman with a 20 year history of lupus erythematosus

          died of sepsis, ARDS, mutiple organ failure and intracerebral hemorrhage.

          Dx: Intracerebal hemorrhang with jaundice.

          A 7-month-old girl developed hyperthermia and shock at age 6 weeks. She

          remain in vegetative state with seizures until her death at age 7 months.

             Dx: Hypoxic-ischemic encephalopathy, remote, with diffuse cortical

          A 48-year-old diabetic woman developed myocardial infarct and

          ketoacidosis 2 months before death . She was readmitted a month

                   infarct           bilateral ACA territories.

          An arteriogram 4 weeks later showed superior sagittal sinus

          thrombosis and left lateral sinus thrombosis,

          died of vaginal bleeding about 2 to 3 months after the onset.

          Dx: multifocal encephalomalacia, venous thrombosis.

          A 67-year-old hypertensive black female developed left "CVA", heart

          block, pneumonia and GI     bleeding after repair of thoracic aortic

          aneurysm. She died 4 months after the incident. The brain   showed

          left cerebral hemiatrophy with cortical pseudolaminar necrosis

          and partially liquefactive of right cerebellar hemisphere.

          A 11-year-old girl showed retardation of both physical and mental

          development since age 2. Terminally, she appeared to be a cachectic

          dwarf with sunken eyes, photophobia,high-pitched   dysarthria,

          flexion contracture of hip, knee and ankle joints, tremor and

          bilateral Babinski's sign.         She died of pneumonia. There were 6 other

          familly members who were affected with the same  condition.

          Neuropathologic diagnosis: Cockayne's disease.

          The wound was debrided but he remained in coma for one year and

          died of pneumonia.

          A 6.5-year-old girl was found to have a brainstem glioma by CT one

          year before her death. She was treated with 7200 rad irradiation,

          high doses steroids and chemotherapy. The tumor continued to grow

          and she was treaed with another load of chemotherapy. She

          developed anemia, neutropenia           and thrombocytopenia and died.

433.   Np 14977

          A 57-year-old man with a histroy of myocardial infarct, coronary

          artery bypass, chronic alcoholim,            rheumatoid arthritis developed

          left hemiparesis and died 6 weeks after the onset of neurologic

          symptoms. Autopsy showed marantic endocarditis and splenic

        infarct.

          Neuropathologic diagnosis: Cerebral infarct due to embolism, right

          MCA and PCA distribution.

434.   Np 15741

          A 10-year-old boy developed ataxia.  MRI showed a tumor in the pons

          extending into the right cerebellum and midbraiN. He was treated

          with 7200 rad irradiation but the tumor continued to   progress. He

          died 7 months after the onset.

435.   Np 15223

          A 81-year-old man had a history of "stroke" on the rigth side of the

          brain 40 years ago when he             was 41. He recovered and become

          symptom-free subsequently. he died of acute pancreatitis.

          Neuropathologic diagnosis: Saccular aneurysm, righr middle

          cerebral artery with old hemorrhage.

436.   Np 16585

          A 58-year-old male who had been diagnosed as haveing AIDS developed

          right hemiparesis and        left homonymous hemianopsia. CT showed a

          low density lesion in the right occipital white matter.

          Neuropathologic diagnosis: Progressive Multifocal

          Leucoencephalopathy (PML)

437.   Np 16852 (Np 15852)

          A 59-year-old truck driver developed a seizure attack while driving.

          A tumor in the left occipital  lobe was found, resected and

          irradiated with I-125 implants.  He did well for 5 months but became

          obtuned. CT showed the lesion to be biparietal. He died 10 months

          after the onset.

          Neuropathologic diagnosis: Glioblastoma multiforme, left

          occiptal.

447.    AU-94-167

          73 year-old male had a history of sensory polyneuropathy for about

          40 years duration.

          DX:  Old cystic infarcts, right superior temporal gyrus and right

                  parietal cortex (incidental).

448.    Np 6151

          Gliomatosis cerebri in 16 year-old female who was diagnosed as

          having grade II astrocytoma 5 yrs     before death. She underwent

          temporal lobectomy 2.5 yrs later and RT with good result but

          recurred      1.5 yrs before death. Chemotherapy did not help.

          Terminally, she could not move and speak and had        seizures.

449.    Np 18403

          5 year-old male born with small head carried diagnosis of cerebral

          palsy, seizures and developmental delay. CT at age of 2.5 mos showed

          asymmetrical ventricular dilatation and several         foci of

          calcification. He was also diagnosed as having CMV encephalitis.

          DX:  right porencephaly and bilateral diffuse micropolygyria.

 450.    Np 18854

          30 year-old female was diagnosed as having multiple sclerosis for 4

          years. She developed GU             infection, depressed and was found dead.

451.    NP 18982

          35 year-old female with 20-year history of lupus erythematosus with

          multiple complications. She     developed mental changes and found to

          be septic. She became comatous. CT showed a cerebral          hemorrhage.

          She died of multiple organ failure. Autopsy showed candida,

          bacterial and CMV        cholecystitis and non-bacterial endocarditis.

          The brain showed left cerebral hemorrhage, old infarcts   and

          multifocal vascular scars.

452.    Np 19477

          40 year-old female developed right arm weakness progressed to

          right     hemiparesis when she was 20       weeks pregnant. CT showed

          multiple      infarcts in the cerebrum and basal ganglia. Arteriogram

          3 weeks   after the onset showed thrombosis of superior sagittal and

          left       lateral sinuses. After the           arteriogram she became

          unresponsive      and died of massive vaginal bleeding about 5 weeks

          after the    onset.

453.    Np 19687  

          67 year-old male was diagnosed as having Huntington’ s chorea 20-30

          years before death. He            developed dementia 3 years prior to death.

       An incidental arachnoid cyst was found on the left            temporal

          convexity.

454.    Np 19777

          35 year-old male was hit in the head with a beer bottle. He became

          lethargic and then comatous.       After CT, a left subdural hematoma

          was evacuated. He remained in come fore 14 months before death.

          Post-operative CT showed left cerebral infarct with edema.

          Additional infarcts in right thalamus and   basal ganglia were found

          3 days later. His hydrocephalus was shunted 1 month prior to death.

455.    Np 20482

          This 38 year-old male had a life-long history of mental retardation

          and seizure disorder. He died of          aspiration pneumonia. The brain

          shows microcephaly(950g), periventricular heterotopia, cerebral

          white matter hypoplasia, hippocampal sclerosis, status fibrosus of

          thalamus and cerebellar atrophy.

456.    Np 20483

          This 63 year-old female presented with lethargy, depression and

          inability of care for herself. She  developed respiratory arrest

          during work-up, resuscitated but remained in coma and died shortly.

          The brain showed multiple metastatic carcinoma with

          intraventricular dissemination and cerebral    edema. Lung primary.

457.    Np 20484

          this 39 year-old male had a history of measles encephalitis at the

          age of 8 when he developed fever        followed by coma 1 week after

          the onset of measles. He remained in coma for 4 months and woke up

          gradually with right hemiparesis and speech difficulty. He improved

          slowly and became able to walk        with braces. Seizures appeared 21

          months later. His mental age was said to be about 20 month-old.

          He developed dilation toxicity 2 years later.  He was paralysed and

          died of seizures when a new anti-        convulsant "Felbamate" was given

          in place of Tegretal and Depakote.

          Brain: 880/79g, marked diffuse white matter sclerosis, most marked in

          left frontal lobe.

458.    Np 20541

          This 1 year and 4 month old girl had a history of malignant rhabdoid

          in the fourth ventricle             resented 8 months prior to her death. She

          died of recurrence.

459.    Np 20542

          This 8 year-old male had carried diagnoses of cerebral palsy, mental

          retardation, developmental          delay and seizure disorder following

          an episode of bacterial meningitis at the age of 6 months. He          was

          deaf on the left ear, blind and had flexion contractures of the upper

          extremities. He died of    ARDS.  

          Brain: 850/65.9g, periventricular heterotopias, large ventricles,

          frontal pachygyria.

460.    Np 20687

          This 53 year-old presented with acute onset of headaches with

          vomiting. CT showed subarachnoid,intracerebral and

          intraventricular hemorrhages. Arteriogram showed an aneurysm in

          the pericallosal            branch of the left anterior cerebral artery. She

          died two days later without surgical intervention.

461.    Np 20935

          This 59 year-old female had a history of AML treated with chemo,

          recurred with pancytopenia          and fever a year later, Lung biopsy for

          infiltrate was negative but biopsy of skin lesion showed

          aspergillosis. CT revealed a right frontal lesion which was resected

          but died 1 week later.

462.    Np 20340 (AH-74-93)

          This 83 year-old female had a history of hypertension, left ACA

          stroke, syphilis and deafness        on the right and died of acute cardiac

          arrest. The brain showed an incidental aneurysm in the left

          vertebral artery at the origin of PICA as             well as left frontal

          watershed infarct and marked atherosclerosis.

          This 46 year-old male was in persistent vegetative state for 10 years

          after an evacuation of      hematoma in the right basal ganglia,

          resulting from an assault. The brain showed an old hematoma cavity

          in the right basal ganglia and internal capsule and           evidences of

          old contusion and hypoxic changes.

          This 52 year-old male developed a sudden onset of headaches

          followed by come. CT and   arteriogram showed aneurysms at

          anterior communicating artery and tip of basilar artery diffuse

          subarachnoid hemorrhage, right frontal hematoma and

          intraventricular hemorrhage.
